Media opt-out selection

By registering for a HOAJC tournament, you understand that certain tournament information may be publicly displayed as part of tournament operations, public reporting, bowler recognition, and the promotion of the HOAJC Junior Classic. Public tournament information may include the bowler's name, home bowling center, scores, standings, tournament results, honor-score announcements, scholarship awards, and other routine tournament information. This information is used for tournament administration, public reporting, recognition of bowlers, and promotional purposes related to the HOAJC. Names, home bowling centers, scores, standings, rosters, results, and other routine tournament information are not included in this media opt-out. If you have a specific privacy concern regarding this information, please contact an HOAJC Director before the tournament. By registering for a HOAJC tournament, you authorize the HOAJC to photograph or record you or your child during the tournament and to use those photographs, videos, images, or likenesses in connection with HOAJC tournaments, including on the HOAJC website, Facebook and other social media channels, promotional materials, and related communications. This form allows you to opt out of HOAJC's use of your own or your child’s photographs, images, likenesses, and video recordings.

Media selection

I understand that the HOAJC may photograph or record me or my child during an HOAJC tournament and may use those photographs, videos, images, or likenesses for HOAJC website, social media, promotional, tournament-related, and other related communications. I understand that this form is used solely to opt out of HOAJC's use of my or my child's photograph, image, likeness, or video recordings. I understand that this opt-out applies only to HOAJC's use of photographs, images, likenesses, and video recordings and does not control photographs or recordings made or used by other individuals, organizations, spectators, participants, venues, media outlets, or third parties. I understand that this opt-out is tournament- and bowler-specific and that a new opt-out form must be completed for each bowler and each tournament for which I wish to opt out. Select the following option if you wish to opt out:
